FATMA involves creating strategic mucosal soft  tissue rearrangements (the "skin" on the inside of the lips) to roll out the lips from the inside to the out. This increases the amount of visible vermilion and to blend it all, fat transfer is performed. He has published this paper in the peer reviewed journal "Plastic and Reconstructive Surgery". It is not only natural but permanent, utilizing your own tissue to achieve the results.
