This patient wanted to look younger and needed advice on how to figure out what was not "working" with his facial appearance. He presented after having undergone many facial injections in the past with filler. The filler gave him excessive cheek fullness that made his eyes smaller and contributed to a disbalance with his long lower face. After detailed consultation, both Dr. HAWORTH and the patient embarked upon a plan to reshape his lower face and oral region, specifically his lip disharmony.
A chin reduction through a supplemental incision (below his chin) to remove both soft tissue and nearly 1 cm of bone was performed. Dr Haworth reattached his chin muscles back to the bone to ensure a youthful jawline in the profile view, otherwise a slack silhouette would result which he has seen in many instances with other surgeons' results. He also performed upper lip lift, medialized external corner lip lift, lower lip V-Y plasty and judicious use of fat and fillers to establish facial harmony across his countenance.
A lower blepharoplasty with fat transfer to the hollows (tear troughs) was also performed to provide a natural and youthful transition from his lower eyelids into his cheek area.Two year follow-up photographs are presented.
