What is a Facelift?

Classic Facelift

With aging comes slackened, buckled skin and a general downward shift in facial tissues as gravity and degeneration take their toll, all resulting in an older, more tired or angrier appearance. When performed properly, a facelift rejuvenates the face and neck by removing selected localized fat bulges (the jowls) while resuspending others (the malar fat pads along with the nasolabial folds) and tightening deeper sagging structures (muscle and connective tissue-SMAS), concluded by a skillful redraping of the skin.

Dr. Randal Haworth can perform one or a combination of different procedures depending on the individual patient, including:

  • A modified, extended facelift which is Dr. Haworth's unique version of the classic facelift (rhytidoplasty, rhytidectomy, meloplasty) dealing primarily with the bottom two-thirds of the face including the neck.
  • Neck lift (cervicoplasty) for those exhibiting loose neck skin with platysmal banding and mild-to-moderate jowls but whose faces do not require the more extensive classic facelift
  • Mid-facelift (subperiosteal endoscopic facelift) for the middle third of the face, that is, the area around the nose, beneath the eyes and around the mouth.

Revision Facelift

Occasionally plastic surgery does not achieve the exact results you had hoped for. You must remember, however, that surgery isn't a means of achieving perfection but improvement. However, if you are unhappy with the appearance of your face after having undergone previous facelift surgery, Dr. Randal Haworth offers revision (or secondary) facelift. Known for his technical and aesthetic precision, Dr. Haworth has extensive experience with revision patients who can expect notable improvement but still must have realistic expectations as to the end result.

When evaluating any potential revision facelift patient, Dr. Haworth will review certain salient details of your previous procedure to determine what may have been done and what went wrong so that he can correct the underlying mistakes. Often times, the cornerstone of this revision facelift procedure usually requires an astute appreciation of skin tension and vectors. Neck Lift or Liposuction

Dr. Haworth notes that a facelift may not only address a sagging midface, troublesome jowls and ill-defined jawline, but it may also dramatically tighten and correct a sagging neck ("turkey gobbler" neck). In the last few years, Dr. Randal Haworth has perfected techniques whereby he can perform a neck lift (cervicoplasty) independent of a full facelift.

A neck lift is relatively easy to recover from and patients can generally return to their normal activities within a week or so. This is ideal for busy professionals, celebrities and others who cannot take much time off from their demanding schedules. Dr. Haworth goes on to say, "What is especially appealing about a neck lift is that it not only tightens and rejuvenates an aging neck but it also reestablishes a youthful jawline, while minimizing the appearance of jowls. Best Candidates for Facelifts

Dr. Haworth makes a point of serving each individual patient's needs and defining overall expectations from any plastic surgery procedure. In simple terms, this allows me to assess whether a patient is fundamentally happy and balanced prior to operating on them."

The best candidates for facelift procedures:
  • Are least 14 years old for young women, 16 for young men
  • Have a well-defined bone structure but whose face and skin are slack and sagging due to age or extreme weight loss
  • Are in good health
  • Have reasonable expectations
A facelift can:
  • Lift sagging cheek skin and accompanying deep soft tissue
  • Soften naso-labial folds
  • Smooth jowls and marionette grooves
  • Tighten and smooth neck and face skin
  • Recontour the jawline and neck
  • Decrease neck bands (platysmal)
A facelift cannot:
  • Correct aging of the eyelids and brows
  • Eliminate all facial and neck wrinkles
  • Rejuvenate the oral region, including sagging of the corners of the lips

Facelift Techniques

Plastic surgery is a field that is constantly evolving as new understanding and ever-more-sophisticated techniques come to the fore. As a respected facial surgery specialist, Dr. Randal Haworth is continually analyzing and critiquing his results and attendant techniques to evolve and provide the best results possible to his patients. Dr. Haworth's facelifts are significantly more sophisticated than in generations past, resulting in far more subtle, natural and yet effective outcomes than ever before.

Firm not Tight
The old 'wind tunnel' effect occurred by pulling and securing the skin without addressing the fat, muscle and connective tissue beneath. By correcting compromised deep facial structure, Dr. Haworth is able to deliver far more natural-looking and long lasting results.

Tailored to Fit
Old techniques relied solely on skin stretching with either no muscle tightening or on maximal muscle tightening with minimal skin redraping. Dr. Haworth feels that there is no one single formula that can be applied to every face. There are many types of faces and many different patterns of facial aging. Some faces are heavy and some are thin; some have "turkey necks" with mild jowls while others have relatively contoured necks but obvious jowls; some skin is thick while other skin is fragile.

Understanding this wide spectrum of facial types has led Dr. Haworth to analyze each face with his multidimensional approach. His unique method of surgical facial rejuvenation is more visual than mathematical. He realizes that the deeper structures must be suspended more vertically (since human faces tend to sag vertically) whereas the skin must be redraped at a slight tangent - a bivector suspension.

Dr. Haworth simply refers to his method as a modified classic or traditional facelift, in which he employs a 270° skin redrape, customized "high SMAS" technique along with judicious defatting of the jowls and a Z-plasty corset platysmaplasty.

Volume Equals Youth
Skin with a plump elasticity and pleasing contour is the most youthful-looking. Previous generations tightened skin without restoring volume. Today, Dr. Haworth spends as much time restoring and repositioning the three dimensional planes of the face and neck as he does to smoothing and tightening the skin.

Single-Stitch Closure
Unlike most facial plastic surgeons, Dr. Haworth takes extra time in closing his incisions both in front of and behind the ear with a single, unseen suture. This can be likened to the thread rendered invisible in the seams of a dress.

Perfect Match
Dr. Haworth calls himself an expert micro-tailor when closing the incisions around the ear. He must expertly trim excess skin to the exact size and shape to conform to the unique nuances of the ear anatomy. Consequently, healing should occur with minimal-to-no tension resulting in near invisible scarring and nearly zero distortion of the hairline and ears. No tension for perfect draping and closure.
